Conflict of Interest
The questions in this section are required to determine whether members of the Research Team, their family members, or the institution are involved in activities that may impose an actual or perceived conflict of interest as it would relate to the conduct of the research or a member's relationship to the Sponsor. All of the questions in this section are required.
Regardless of what you select for each question, you will be able to submit your proposal for routing. If you think a conflict exists, please refer to your institution's policy on conflicts of interest and commitment.
Once you are finished answering yes or no to all questions within the Conflict of Interest section, click Save.
Questions
- Does this project have any potential real or perceived conflict of interest as defined by A&O Manual Policy 1.12 or 2.10 and the federal agency or sponsor to which this application is being submitted. By checking "No", you attest that you have complied with the University’s Conflict of Interest Policy 1.12 and 2.10 and that you do not need to provide disclosure of either a conflict of interest or commitment.
- Publicly Traded Entities, payments or value exceeding $5,000 when aggregated for an Investigator and the Investigator’s spouse/domestic partner and dependent children from a rev. 1/20/2016 2 of 3 single entity, including salary, consultant payments, honoraria, paid authorship, equity interest (stock, stock option or other ownership interest) during the prior 12 months.
- Privately Held Entities, payments or value exceeding $5,000 when aggregated for an Investigator and the Investigator’s spouse/domestic partner and dependent children from a single entity during the prior 12 months or when the Investigator and the investigator’s spouse/domestic partner and dependent children hold any equity interest (stock, stock option, or other ownership interest).
- Intellectual Property, intellectual property rights and interests (patents, copyrights_ upon receipt of income related to such rights and interests.
- Travel Reimbursements, any reimbursed or sponsored travel related to the Investigator’s Institutional Responsibilities during the prior 12 months, other than that sponsored by a Federal, state, or local government agency, an institution of higher education, an academic teaching hospital, a medical center, or a research institute that is affiliated with an institution of higher education.
