While building a form, you have many objects, or question types, to choose from. Which object used is dependent upon the type of question asked.
Significant Financial Interest

This is the only object that is required when placed on the disclosure form. Here, researchers can use the external organization finder to find external organizations within your Cayuse database. Researchers can add as many external organizations as they need to add.
Attachment Point

Attachment points allow researchers to upload files relevant to their COI form.
Check Box

With check box objects, you can provide a multiple-choice question with many answers for the researcher to choose from. With this question type, the researchers can choose as many answers as they wish.
Currency Input

If your form needs a question where the answer is currency, such as a salary listing, you can use the currency input.
Date Input

If a form question requires keeping track of dates and periods, you can use the date input object. If your question needs a start date and an end date, you can add them separately and place them next to each other.
Dropdown

Dropdown objects allow researchers to select one answer from a drop-down list. You can add as many answers to the question as you need to.
Information

Information is a static object, and the user does not interact with it. You can use it if you need to add information into the form that isn't a question description or help text.
Numeric Input

By using the up and down arrows, researchers can input a numeric value as an answer to the question.
Radio Button

Like check box objects, radio button objects are a multiple-choice question with many answers for the researcher to choose from. However, with this question type, the researchers can only choose one answer.
Rich Text Input

The rich text input object provides a text box that the researcher can use to fill in their answer. The box has no character limit, and provides both font customizations and an option to hyperlink.
Table

A table allows researchers to input multi-object responses. Learn more about tables here.
Text Input

The text input object is for short answers.
Finders

Finders allow researchers to search for people, organizations, or units within the Cayuse database. With finders, users don't have to manually input data, so there is less of a chance for errors.
