Incident Description
On Tuesday, June 23, 2026, we received reports of various issues that were impacting the Graduate Education Manager (Ph.D. Manager) and Ethics Monitor applications. Affected users were either unable to access the application(s), were experiencing missing links within the user interface (UI) and/or were unable to access documents within.
This was immediately escalated internally as an Urgent priority/Severity 1 issue.

Root Cause
We identified the root cause as an unnecessary configuration step (specifically a mount command) in our EFS (Elastic File System) runbook.
Resolution
The issue was resolved by redeploying plugins, which restored missing links within the application UI and addressed any access issues.
Future Mitigation
Cayuse performed the following step(s) to prevent any reoccurrence:
- Audited runbooks and implemented an additional verification and confirmation guardrail.
