Incident Notification
On Tuesday, February 24, 2026, at 8:27am PST, we began receiving reports of intermittent access issues, with affected users unable to log into the Human Ethics application and encountering error messages. Users also reported performance degradation when the application was accessible, resulting in reduced system responsiveness and usability. We sincerely apologize for these disruptions and any impact this may have caused.
Resolution
The issue was resolved at approximately 12:00pm PST on Wednesday, February 25, after which the application resumed normal functionality and performance. No client data was compromised, accessed, or lost during this incident.
What We’re Doing
We have identified the root cause and both the Infrastructure and Product Engineering teams are implementing safeguards to prevent this issue from recurring.
