On Monday, February 16, 2026, at 5:30am PST, we identified a display issue within the Human Ethics application affecting user email fields. In certain instances, placeholder email values appeared in place of valid user email addresses. The issue was isolated to the user interface and did not affect underlying data integrity.
We sincerely apologize for the disruption and any impact this may have caused.
Resolution
The issue was resolved at approximately 4:00pm PST on Monday, February 16, after which the application resumed normal functionality. No client data was compromised, accessed, or lost during this incident.
What We’re Doing
We have identified the root cause and are implementing additional Product Engineering safeguards to prevent this issue from recurring.
