While building your form, you may choose to add 'Question' or 'Information Text' Form Elements. When you add a Question, you will need to select a question type.
Choosing a Question Type
Within the question element, click the down arrow (circled below) under Question Type to select an option from the dropdown list.
When making changes or updates, be sure to save your work by clicking on Save Draft.
Understanding Question Types
Text
The text question type is for short answer responses.
Rich Text
The rich text question type provides a text box that the researcher can use to fill in their answer. The box has no character limit, and provides both font customizations and an option to hyperlink.
Textarea
The textarea question type is for long answer responses with no formatting.
Date
If a form question requires keeping track of dates and periods, you can use the date question type. If your question needs a start date and an end date, you can add them separately and place them next to each other.
Number
Researchers can input a numeric value as an answer to the question.
Radio Buttons
Radio button question types are multiple-choice questions with many answers for the researcher to choose from. With this type of question, researchers can only choose one answer.
Dropdown
Dropdown questions types allow researchers to select one answer from a drop-down list. You can add as many answers to the question as you need to.
Attachment
Attachment question types allow researchers to upload files relevant to their SP 4.0 form.
Checkboxes
With checkbox question types, you can provide multiple-choice questions with many answers for the researcher to choose from. With this question type, the researchers can choose as many answers as they wish.
Person, Unit, or External Organization Finder
Finder question types allow researchers to search for people, external organizations, or units within the Cayuse database. With finders, users don't have to manually input data, so there is less of a chance for errors.

Information Text
Information Text elements are static, and the user does not interact with them. You can use them if you need to add information into the form that isn't a question, description, or help text.
